STLTH Product Return Form

Welcome to the new LKVape RMA application form for STLTH products.

Instructions

⚠️Please read carefully.

This form is to be used to return RMA STLTH products to the LKVape warehouse for credit to your account.

All products requested must be physically received at the LKVape warehouse before any credit can be issued.

You will be able to submit this form to LKVape by using the button at the bottom of this page.

You will need to ship your returned products to our Boisbriand, QC warehouse at the following address:

To ensure delivery, please choose a shipping method that includes tracking and keep the tracking number until we confirm reception of your products.

⚠️ Please note that ALL products must have the federal or provincial excise stamps (except STLTH device).

Please fill out the form below (click 'Add' to add a line) and list all of the products you wish to submit for a return authorization.

You will need to submit photos of all of your devices next to their boxes, with the following information clearly visible on the photos (you can have multiple products in a photo, as long as the requested information is clearly visible) :

• Device
• The device's box
• The federal excise tax stamp (with the visible number)
  
• The flavor and nicotine level of the device

By submitting this form, you confirm that all information provided is accurate and that the listed products are included in your shipment.
