Food items RMA application form

Welcome to the new LKVape RMA application form for food products.

This form is to be used to submit a food product that is improprer for sale in order to get a credit. Please have your products on hand with the original packaging and fill the form. You will need to upload a picture for your items.

Please keep the products until we confirm your RMA application and credit has been issued to your account.

💾 At any time while filling this form you can click the "Save" button to send an e-mail with a link to your inbox so you can resume the return form at a later time. While you will be asked if you want to create an account, note that there is an option to skip this step.

Please fill out the form below (click 'Add' to add a line) and list all of the products you wish to submit for a return authorization.

You will need to submit photos of all of your products next to their packaging, with the following information clearly visible on the photos (you can have multiple products in a photo, as long as the requested information is clearly visible) :

• Product
• Expiration date
• Barcode

You may submit up to five (5) products per form.

 

N.B: Please be informed that we no longer accept products if the expiration date has passed.

Please make sure that all of the product and packaging is visible on the pictures you upload.
